Patient Information

New patients are always welcome at The Toorak Clinic.

On making a first-time booking, you will be asked to arrive 10 minutes prior to your appointment time in order to complete a patient medical questionnaire. This allows the doctor to understand your overall health background to assist with your diagnosis.

You can save time by printing out the Toorak Clinic Health Questionnaire (click on this link) and filling it out prior to arriving at the clinic.

If you choose to switch from another clinic, a proforma is available that can be signed and sent off to your previous doctor requesting transfer of your medical records.

Consultations are by appointment only; however, we will attempt to fit in patients who require urgent attention.

Prescriptions:

Prescriptions are available either by appointment or by phone when a fee may be charged.

Test Results: 

Test results can be followed through by consultation or telephone.

Fees

Accounts are expected to be paid on the day. Doctors accept Visa, Mastercard, AMEX and Eftpos. Doctors consultations fees are generally as follows, but because Toorak Clinic doctors are independent of one another these fees may vary.

Repatriation Patients:

Repatriation patients are billed to DVA to avoid out of pocket costs.

Health Care & Pensioner Concession Card holders: 

At the doctor’s discretion, patients who hold either a Health Care or Pensioner Concession card may be billed directly to the government (bulk billed) to avoid out of pocket costs.

Rebates: 

Medicare rebates can be electronically claimed at the times of payment. This will be paid directly into your nominated bank account by Medicare.

New Doctor consulting at The Toorak Clinic. RSV, Covid–19 Vaccination Guidance, and Sunday and Public Holiday Appointments

If you have symptoms of a Cold or Flu, please do a RAT test before attending the clinic, and advise Reception that you have  symptoms of respiratory illness when booking an appointment. Please wear a mask to minimise spread of your illness.

If you are over 60 years of age, ask your Doctor about the new RSV (Respiratory Syncytial Virus) vaccine and whether it may be recommended for you.

The Toorak Clinic continues to administer Covid-19 vaccination boosters. Please note the COVID-19 boosters are only administered in our mini COVID Clinics : please phone Reception to make a booking on 9827 3600. The Monovalent (Omicron-specific) JN-1 vaccine has been available since late- December 2024 and is available to everyone aged 18 years and over provided that they have not had a Covid infection or vaccination within the previous 6 months. It is a booster vaccine and therefore is not appropriate for those who have not had a primary course of two vaccines (three vaccines if immunocompromised). ATAGI recommends this booster be given to all adults aged 65 and over and all adults aged 18 to 64 who have a pre-existing health condition that makes them more prone to serious illness if they contracted Covid-19. It is also available to all adults aged 18 to 64 who are otherwise well but who choose to be vaccinated.
